By filling out and submitting this Membership, and/or by attending ANY AND ALL Premier Junior Tour tournaments in
2010 and all future years, both competitor and any and all spectators hereby agree to the following STATEMENT OF
RISK LIABILITY AGREED TO BY BOTH PLAYER AND PARENT(S)/GUARDIAN(S):
I accept that there are certain risks inherent in the game of golf, and I accept personal and sole liability for all such
risks. In consideration of the member’s participation in PREMIERJUNIORTOUR.COM, LLC (hereinafter referred to as
“Premier”) membership and events, the junior tour member and the parent(s)/guardian(s) hereby release Premier, the
Junior Golf committee, the Tour’s sponsors, host sites of Premier events, and all of each company’s respective officers,
directors, employees, agents, volunteers, subsidiaries and affiliates from any and all liability associated with either acts
or omissions of such parties or with accidents or injuries sustained by the member or parent(s)/guardian(s) in connection
with the tournaments. Also by checking this box, the player, the player’s parents, guardians and anyone representing the
player in any way, agrees to abide by all tournament regulations with the understanding that tournament entry fees are
non-refundable within time frames as indicated on the Premier website. In case of medical emergency at a Premier
event, the tournament participant and parent(s)/guardian(s) hereby authorize any and all appropriate treatments as
determined by qualified and licensed medical personnel. The member and spectators also hereby grant and assigns
his/her individual media/photo rights while partaking in all Premier events. STATEMENT OF ACCEPTANCE,
AGREEMENT & AUTHORIZATION: By filling out this membership form, we agree to all Premier rules and policies as
published on the website: premierjuniortour.com. The competitor hereby agrees that he/she is subject to
rejection/disqualification at any time (including during the Tour Championship) by Premier staff and reasons for
rejection may include improper attire and/or unbecoming conduct. I agree to observe all regulations and conditions as
stated here and on the website, as well as at any tournament. No child under age 18 may submit an entry to Premier,
online or otherwise, without parental consent. I understand that this entry form is to be submitted only with involvement
of a parent or guardian. We agree that any credit cards or checks used for tournament entry of this player are done with
legal authorization.

BY CLICKING THE SUBMIT BUTTON BELOW AND/OR PLAYING AT ANY PREMIER EVENT, I/WE HEREBY ACCEPT THE
ABOVE TERMS.

IMPORTANT TOURNAMENT INFORMATION:
After you submit your membership form by clicking the "Submit" button below, you may sign up for as
many tournaments as you wish by clicking on the Schedule page.

Tournament Sign up options: 1) Credit card, debit card or set up your checking account for withdrawals.
Sign up online from the Schedule page. 2) Payment by Check: email karen@premierjuniortour.com and
indicate tournament(s) you wish to sign up for. Then mail a check before the event to: Premier Junior
Tour, 845 SE 3rd Place, Cape Coral, FL 33990. Checks will not be deposited prior to 14 days before an
event.

Any player, any level, can join. We will work with you to help you grow and learn.
